Our direct client, Gavin Eldercare Management, LLC provides professional care management services to support patients and families in navigating the complexities of short- and long-term medical care. Our team delivers comprehensive assessments, develops individualized care plans, and coordinates services across hospitals, skilled nursing facilities, assisted living communities, and home-based settings. We also support patients transitioning to palliative and hospice care. Utilizing a holistic, client-centered model, we offer evidence-based guidance that facilitates informed decision-making, ensures continuity and quality of care, and promotes optimal outcomes for patients while reducing stress and time burdens on family caregivers.

 

Responsibilities:

Under the supervision of the Nurse Navigator Leader:

  • Visits clients in the hospital, assisted living community, rehabilitation facility, and/or home

  • Performs comprehensive assessments and develops individual care plans

  • Maintains detailed records of each client visit

  • Communicates on an ad hoc basis with medical specialists and family members

  • Arranges for comprehensive in-home care

  • Attends medical appointments as needed in the capacity of client advocate

  • When necessary, transports client in RN Care Manager's personal vehicle

  • Provides ongoing support and empathy to clients and their families

  • Provides crisis intervention

  • Reports on a daily basis to the Nurse Navigator Leader and when necessary, more frequently

  • Remains up to date on best practices and information related to the healthcare concerns and conditions of our aging population

  • Caseloads vary and may range between 1 - 3 clients/day dependent on clients’ current needs and your overall availability

  • Hours: 8 - 15 hours/ week – negotiable / flexible

  • Technical skills: MS Word and/ or Google Docs preferred

 

Experience:

  • 2+ years of geriatric nursing experience

  • General understanding of the major issues of aging (loss, grief, physical changes, depression, dementia)

NOTE: This is not a hands-on nursing/medical position. Your function is to provide guidance that is in sync with the client’s medical team.

 

Licensure:

Active State of Connecticut RN License (preferred)

 

Education:

Bachelor of Science in Nursing (preferred)
